Veterinary Referral Form

In order to visit Scuba Sam’s facility we require for the form below to be completed by your primary veterinarian.

This form will confirm that your pet is in fact a suitable candidate for hydrotherapy, use of our spa, laser and shockwave therapy.

Along with this form, your veterinarian will send us your dog’s individual clinical history (where applicable) so that this can be reviewed prior to your initial appointment.

Remember that canine hydrotherapy is suitable for most dogs, for general fitness, conditioning and helping with various conditions - your vet will be able to tell you either way.

*Due to Texas laws and regulations, you must complete our veterinary referral form and send it in to us before we can book an appointment for you.
